Primary Activity: the main activity of the group.

• Backcountry Tourer: use mostly human power to ascend, and can be on skis, splitboards, or snowshoes carrying tools to make turns with.
• Climber: focused on route, summit, or peak, often technical
• Hiker: on foot, no technical routes
• Hybrid Rider: use snowmachines to access terrain for making turns on. In the narrative, please note if they were caught while on sleds, skis, or snowboards.
• Sidecountry Rider: use ski areas for much of ascent, leave boundaries, and plan to return to the area.
• Inbounds Rider: Within operating ski areas.
• Snowplayer: sledders, tubers, winter campers, or others playing in the snow.
• Misc Recreation: recreation not covered in other catagories.
• Mechanised Guiding Client: client of heli or cat operation.
• Human Powered Guiding Client: guided client, not using mechanized transportation.
• Mechanised Guide: guide of heli or cat operation.
• Human Powered Guide: guide not using mechanized transportation.
• Ski Patroller: caught on duty.
• Ranger: USFS or park ranger caught while on duty.
• Highway Personnel: caught on duty. In the narrative, please note if they were caught in vehicles or on foot.

Elevation, Slope Angle, Aspect: of the starting zone.

Type, Trigger, Size, Surface: Follow SWAG standards.

Depth, Width, Vertical: Provide average or typical dimensions. Maximum and minimum values can be noted in the Comments.

Number Caught: People caught but not buried by the avalanche.

Injured: Please note extent of injury in the narrative, without infringing on patient's rights.

Partially Buried, Non-Critical: Buried with their head and/or other body parts above the snow surface.

Partially Buried, Critical: Buried with their head below the snow s urface, but other body parts above.

Buried: The number of people fully buried. Note in the narrative if the burial was partial, with their head above the snow.

Killed: People killed by the avalanche. If you are an avalanche professional, please use the Investigation Form for fatalities.

Comments: Other pertinent details of the avalanche. There is a 8000 character limit.

Accident Links: URLs with additional information, photos, weather history, forecasts, or the like.

These fields are for the event narrative. DO NOT use names of those involved. Numbers or initials are apropriate.

Confidential Comments: These comments will remain internal and seen only by avalanche center personnel or researchers. Please add any pertinent details.

Avalanche Education: Subjective rating. In general.

• None: No avalanche knowledge, often unaware of avalanches.
• Some: Some classes (Level 1) or experience. Aware of avalanches and avoidance.
• Advanced: Formal classes (Level 2) and or experienced.
• Expert: Avalanche professional or highly skilled and experienced individual.

Involvement Type: These fields allow you to specify the individual's involvement in the avalanche.
